Case Manager
Care Coordination – Brainerd, MN

Northern Pines Mental Health Center is seeking Case Managers to join our Care Coordination Division. Full-time positions are available in both Brainerd and Little Falls, MN. The general purpose of this position is to provide Targeted Case Management to consumers in a community setting. Qualified candidates must be able to work well in a team setting, practice discipline in independent work, and express enthusiasm for building and retaining community relationships. Duties will include but are not limited to: developing, reviewing, and incorporating treatment plans based on the functional and diagnostic assessments; planning, assessing, record keeping and documenting associated with the service coordination of a consumer; completing and regularly reviewing the written functional assessment; assisting consumers in obtaining needed services; coordinating with other agencies to obtain/coordinate/monitor services for consumers.

This position may be filled by a mental health practitioner as defined by the State of Minnesota. A Bachelor’s degree as well as experience providing chemical health services is preferred. Travel is a function of this position; a valid driver’s license and ability to operate a motor vehicle is required.
