BHH Systems Navigator
Behavioral Health Home

Northern Pines Mental Health Center is seeking a BHH Systems Navigator to join our Behavioral Health Home team.  This full-time position will primarily be located in Brainerd, MN with travel throughout the region 5+ service area. The purpose of this role is to coordinate, monitor, and refer to services to meet the behavioral health and physical needs of consumers and works closely and effectively with the Behavioral Health Home Team. This position requires that the employee must maintain an excellent working relationship with the BHH team and community contacts to include but not limited to staff within medical facilities, county contacts, and agencies within the service area providing resources to BHH consumers. This position requires verbal and written communication skills. Applicants must qualify as a Mental Health Practitioner as defined by the State of Minnesota. A Bachelor’s Degree in the behavioral sciences is preferred. A valid driver’s license and ability to operate a motor vehicle is also required.
