Safe Harbor

Safe Harbor provides supervised adult crisis residential stabilization service to Region 5+.  Safe Harbor is a six bed supervised residence in a tranquil setting in Brainerd, MN.  The crisis residential services are short term services for adults, 18 years of age and over, who are currently experiencing a mental health or dual diagnosis crisis. We are not a detox facility.

Safe Harbor has 24 hour/7 days a week staffing that provides assessments of immediate needs and assists consumers to identify factors that may have led to the crisis. Consumers develop goals to assist in crisis reduction so they can return to baseline. Safe Harbor staff will make and follow up with referrals for identified supports. Safe Harbor has nursing staff and psychiatric consultive services.

  Who is Crisis Stabilization Service for?

Services are designed for adults 18, and over, who are experiencing a lapse in their functioning due to an increase in their mental health symptoms. Individuals challenged by a serious mental illness are often vulnerable to homelessness, abuse of alcohol/drugs and legal issues.  Safe Harbor provides a supervised residential rehabilitative service on a time limited basis with referrals for other services.

Crisis Stabilization Services provide a safe supportive environment and staff for a consumer to return to baseline after a crisis. Services will include an immediate needs assessment, identifying the factors that lead to the crisis, treatment plan, supportive counseling, skills training, discharge planning and collaboration with family and other service providers.

  Team Approach
  The Crisis Stabilization team consists of a consulting psychiatrist, nurses, mental health professionals and practitioners, as well as certified peer specialists. The team will collaborate with family and current social service providers.
  Continuous Care
  Consumers will work with the team members who have the best skill at any given time to provide the support needed to reach their goals.
  Personalized Services
  Crisis Stabilization Team work with a relatively small number of consumers in a peaceful setting, thus providing personalized care based on their treatment plan and receive an intensive level of individualized support.
  Crisis Stabilization Services is designed as residential setting so and consumer can recover, develop new skills and prepare for a transition back to the community in a safe, supportive environment.  The services are delivered in a tranquil residential setting with supportive staff on a 24-hour basis.

Crisis Stabilization nursing staff will assess each consumer to determine their level of services needed for medication administration and health care service delivery.

  Family Involvement

Crisis Stabilization Services respect the wishes of the consumer whether to involve the family in treatment.  The team will offer family support and education so there is a better understanding of mental illness and the recovery process.


Psychosocial Treatment


Illness management and recovery (IMR) skills assist consumers to recognize the symptoms of the mental illness they experience. The factors that may contribute to “trigger” the onset of symptoms will be explored. Another focus of IMR is warning signs and relapse prevention planning. Problem-solving, goal-setting and stress management skills are an integral part of illness management and recovery.

Integrated Dual Disorder Treatment in screening, assessing, treatment readiness, use of motivational interviewing and employs harm reduction strategies for treating chemical dependency.

Family Education services to educate, inform and support family members in mental illness and treatment.

Please call the Crisis line at 1-800-462-5525 (or) 218-828-4357 and request a referral for Mobile Crisis Outreach services.

Mobile Outreach services will screen adults, 18 years of age and over, who are experiencing a mental health crisis for admissions. Safe Harbor also accepts step down admissions from hospitals if consumers no longer meet admission criteria.  Please call Safe Harbor director for additional information.
